CEU Log Form To maintain your CTR credential, it is imperative that you maintain a CEU LOG each year to account for 20 clock hours of approved seminars, meetings, etc. Below is a link to a CEU SUMMARY FORM in the PDF format to assist you in correctly retaining and logging the data for meetings and seminars for each year. It is NOT the actual form you will submit to the NCRA each year, but a diary of sorts to help you jog your memory. To obtain this form requires that you have Adobe ACROBAT Reader installed on your computer.
